Membership and Directory Listing Form

To join the Dealey PTA, fill out the online form below or download, print and submit a completed form along with payment to your child’s teacher.

The online form allows you to pay by credit card through our secure Paypal account. The Dealey PTA does not have any access to your credit card payment information.

If you would like your family’s contact information to appear in the Dealey Directory, but do not wish to join the PTA, complete this form and submit it to your child’s teacher.

Please Note: For privacy purposes, any individual or family requesting a directory must have a family member that attends Dealey or be a Dealey employee.

NEW THIS YEAR: YOU MAY CHOOSE ELECTRONIC COMMUNICATIONS FOR ANY OF YOUR CHILDREN INSTEAD OF RECEIVING HARD COPIES IN THE THURSDAY FOLDER.
